UP's AI guide for lecturers says students complete the University's Generative AI declaration form with each assignment, whether or not they used AI. Fill in Q1 to Q4 here and copy them; Q5 and Q6 ask for your own reflection, so the generator leaves them for you.

Who needs it
UP's AI teaching and learning guide for lecturers (Version 9) says students must complete the form upon submitting each assignment, regardless of whether any AI tools were used, and asks lecturers to state this in the study guide and assignment briefs. If you tick option 2 in Q2, the form stops there.
What the generator fills in, and what it leaves to you
It fills Q1 from what you type, Q2, a Q3 row for each prompt you paste (the aspect of the assignment is left for you) and Q4, with your uses matched to UP's six features and Not Applicable for the rest. It leaves Q5 and Q6, because they ask for your own reflection.

Q3 asks for all your prompts. The UP Library's AI and Academic Integrity checklist (updated 4 February 2026) asks: Have I saved all prompts used with AI tools for potential lecturer review if requested to demonstrate appropriate use?

Questions
Do I have to submit the UP AI declaration form if I did not use AI?
Yes. UP's guide for lecturers says students complete it with each assignment regardless of whether any AI tools were used. Tick option 2 in Q2 and the form stops there.
Do I have to paste every prompt in Q3?
Q3 asks you to paste all the prompts you created for the assignment and say which aspect of the assignment each was for. Paste them into the generator one per line and it lays them out by tool; add the aspect yourself.
Can the generator write my Q5 ethical use statement?
No. Q5 asks how you made sure your use of Gen AI was ethical and responsible, and Q6 asks why you used it. Those are your own reflections, so the generator marks them for you to write.
Where do I get UP's own copy of the form?
It is Appendix B of UP's AI guide for lecturers, linked below, and your study guide or assignment brief should state it. If your module gives its own copy, copy your answers onto that.
