Monash AI Acknowledgement Statement Generator
Monash suggests one sentence per tool: the tool and its link, how you used it, the number of iterations or drafts, and how you modified the outputs. Fill it in here, or get Monash's recommended wording if you chose not to use AI. Your Chief Examiner's instructions come first.
Checked 4 October 2026 against Monash's AI hub page on acknowledging the use of AI. If your module gives its own form, use that.

OpenAI. ([year]). ChatGPT [Large language model]. https://chatgpt.com/
In the text: (OpenAI, [year])
APA (September 2025): no version number by default, and the year is the year the tool was last updated; if you are unsure, use the year on the tool's site. A share link and a chat description switch this to APA's format for a specific chat. Monash's AI hub gives this format:
I used [add AI tool (add link if needed)] to [add how used] and [add number of iterations/drafts]. I modified the outputs in [add ways].
Its example acknowledgement section is headed Part B: Concise summary of AI use in the assessment, and asks you to explain which tools you used and for what, what output was included and where, why you chose any tools you found yourself (with a link), the number of iterations with each main tool, and how you altered, adopted or built on the output.
If you chose not to use AI
Where AI was permitted but you did not use it, Monash recommends: No content generated by AI technologies has been used in this assessment.
Documenting your AI use
Monash suggests screenshots of different stages of your AI use, copies of prompts and outputs, copies of drafts and notes on how prompts were integrated and adapted, and colour-coding or annotating drafts where AI was included.
Your Chief Examiner comes first
For each assessment, your Chief Examiner should explain how to acknowledge AI use. It may be a written section, screenshots or a video, and may include a reflection.
Referencing AI at Monash
Monash asks you to cite and reference AI output when necessary, using the closest source type in your referencing style. Its APA example dates from 2023 and includes a version number. APA's September 2025 guidance no longer adds one by default, and the generator follows it; use your unit's style guide if it differs.

Questions
Is this the Monash Generative AI declaration form?

What do I write if I did not use AI?
Monash recommends: No content generated by AI technologies has been used in this assessment. Choose No in the generator to get it.
What counts as an iteration?
Monash asks for the number of iterations undertaken with each main AI tool, for example how many drafts or rounds of prompting. Its own examples write it in brackets, such as (2 iterations).
How do I reference ChatGPT at Monash?
Monash asks you to cite and reference AI output when necessary, in your unit's referencing style. The generator gives APA, Harvard and MLA following each style's current guidance.
